What is Proxy Access?
Proxy Access is when patients choose to give another person access to their online GP record. This is usually a family member, next of kin or a carer.
A Proxy may be chosen for many reasons, including:
Providing better information to family members and carers about your medical conditions
Giving someone access to your prescriptions record, so they can understand what medications you need and how much
To set up a proxy with us, please visit the Surgery in-person to collect the required form. We also require a signature from the patient in question to provide consent.
